If you or a loved one would like to attend the Faulkner University Center for Therapy and Research, please follow the simple steps below or call if you have questions.
Step 1
Ensure you meet our eligibility criteria and understand our firm attendance policy.
The Faulkner University Center for Therapy and Research is a grant-funded teaching clinic that depends on client attendance to continue to operate. As such, we work to ensure that our students can achieve optimal learning experiences to further their clinical education, as well as provide the highest quality services for each of our clients. Consistent client attendance is of the utmost importance to achieve the best outcomes and continue our clinic operations. Because of this, a strict attendance policy is in place.
Step 2
If you are requesting Speech Therapy, Occupational Therapy, or Physical Therapy services, have your physician fax the referral requesting evaluation for the specific discipline to 334-386-7354 or upload it under “documents” via the patient portal. If you are requesting Mental Health Counseling, no referral is needed. Simply select the Faulkner Mental Health Center within the patient portal or give us a call to request an appointment.
Step 3
Request an evaluation by completing the patient portal. The portal will expedite your registration process and you will be contacted to select an initial evaluation time or place on our waiting list. If you are not able to complete the patient portal, please give us a call to request an evaluation. New patient forms are available for you to print and complete manually before your first visit.
Step 4
Prepare for your first visit. Please arrive 15 minutes early to sign-in at the reception desk and complete any additional intake paperwork before your appointment. Your clinician may have questions for you and can assist if needed.
